The fourth of six stories from the Kowai Fairy Tale series (コ ワ イ 童話). Each of them is a loose and macabre adaptation of one of the classic fairy tales. This story adapts a fairy tale ‟The Ugly Duckling‟. Yuma, a young boy with mild anemia, is brought to the hospital where a new nurse, Ibuki Miyamoto, works. Ibuki tries to contact his parents, but no one answers the phone, When she decides to show up at the boy's house alone, she discovers that his parents vomit worms and green goo.
